This position coordinates medically related social services to Sub-acute and nursing home residents. Duties include:

• Care planning

• Discharge planning

• Advocating for residents’ rights

• Providing emotional support to residents and their family members.

Qualifications:

• Must possess a bachelor’s degree and a CSW or eligibility for CSW.

• Experience working in a nursing home setting.

Preferred experience:

• Nursing Home Social Service: 2 years
