Position Description:
To perform nutrition assessments and reassessments, determine medical nutrition therapy goals and desired outcomes,
develop and implement nutrition care plans and nutrition interventions, provide nutrition education, and record timely and
appropriate documentation in the medical record. Work involves planning, organizing and directing the operational
activities of ambulatory nutrition services in collaboration with other care team members and nursing leadership.
KEY RESPONSIBILITIES
1. Participates in community outreach programs as it relates to health, diet/nutrition, and wellness.
2. Develops, implements, and monitors patient’s nutritional care plan based upon diagnosis, prescribed treatment, and
diet history.
3. Follows all applicable facility, professional standards, policies, and procedures.
4. Educates the patient and/or caregiver, dietetic interns and students, other care team members, community
representatives on nutrition, nutrition-related topics, and/or designated drug-nutrient interactions.
5. Participates on committees and attend meetings such as primary care providers, quality improvement, patient safety,
and interdisciplinary meetings.
6. Provide 1:1 education and group style teaching to patients.
7. Assists in crafting clinic and facility policies and procedures.
8. Supports quality and fiscal goals for the department and institution, assists in inventory management, assists with
CQI projects, and identifies obstacles to improvement within the clinical area, assists with policies and procedures
development for the facility.
9. Participates in special projects that supports the operation of the organization and other duties as assigned.
Position Requirements:
Position requires bachelor’s degree in Dietetics And (2) years of related work experience required. Work experience may
NOT substitute for minimum education requirements.
Heartsaver certification is required prior to hire.
Currently license as a Dietitian with the State of Alabama Board of Examiners for Dietetic/Nutrition Practice required.
