COPC Brand is Hiring a Community Health Worker Near Westerville, OH

Central Ohio Primary Care is seeking a Community Health Worker for our central business office in Westerville, OH. The Community Health Worker (CHW) serves as a link between health care providers, care coordinators, community service providers, and individuals in the community to promote health, and address social determinants of health. The CHW builds individual and community awareness by increasing health knowledge and self-sufficiency through a range of activities, such as outreach, community education, social support, and advocacy. The CHW identifies barriers to health and advocates for strengthening service delivery and other strategies to improve population health or individual well-being. At the direction of a licensed social worker or registered nurse, the CHW will assist care coordinators with accessing appropriate resources, coordinating referrals as necessary, providing community and educational support, and serving as a community liaison. This is a full-time position working Monday- Friday 8a-5p. Some travel in a specific area of Central Ohio will be required.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Under the direction of a licensed social worker or registered nurse, will assist care coordinators with accessing appropriate resources, coordinating referrals as necessary, providing community and educational support, and serving as a community liaison through telephonic outreach or in-person visits in homes, provider offices, or public agencies.
  • Create connections between vulnerable populations and healthcare providers
  • Help patients navigate healthcare and social service systems
  • Reduce social isolation among patients
  • Determine eligibility and enroll individuals in health insurance plans
  • Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity, and nutrition at the direction of the registered nurse
  • Advocate for underserved individuals or communities to receive services and resources to address social determinants of health

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

A. Experience, Education, Licensures & Certifications 

  • Required: Minimum of 3 years’ experience in home health, hospice, managed care, or social service agency. Prior pediatrics background preferred, but not required.
  • Required: HS Graduate or Equivalent
  • Required: Active driver's license and ability to drive in Ohio
  • Preferred: Licensure/Certification: Community Health Worker certification

B. Knowledge, Skills & Abilities

  • Proficient in computer software and usage including but not limited to Microsoft Outlook and Microsoft Excel
  • Knowledge of community resources and public health agencies
  • Strong interpersonal and problem-solving skills 
  • Knowledge of motivational interviewing techniques
  • Ability to work independently and in a team environment
  • Demonstrate self-awareness and cultural sensitivity
  • Ability to maintain professional and ethical boundaries
