Cori Manor Healthcare and Rehabilitation Center is Hiring a MDS Coordinator Near Fenton, MO

We are seeking a compassionate and dedicated MDS Coordinator to join our healthcare team.

An MDS coordinator, or Minimum Data Set coordinator, works in healthcare facilities to gather information on patients' physical and mental health for future assessments. They also create care plans for current and incoming residents, and ensure that the quality of care is coordinated from pre-admission to discharge. Here are some of the responsibilities of an MDS coordinator:

  • Assess patients: Perform initial interviews and assess new residents, as well as evaluate the care of long-term patients
  • Create care plans: Communicate with healthcare teams to develop plans that are applicable to each resident, and ensure that they comply with ethical standards and Medicare requirements
  • Follow up with patients: Ensure that patients receive proper care, and follow up with them on any complaints or incidents
  • Meet with families: Discuss the patient's condition and treatment plans with their families and caretakers
  • Monitor payments: Keep track of patients' medical insurance and payment plans
  • Host meetings: Discuss patient care and implement new strategies

MDS coordinators should have a thorough understanding of the MDS assessment process, including its guidelines, regulations, and updates. They should also be familiar with coding resident assessments accurately. Other skills that are useful for MDS coordinators include:
