Title: MAT Coordinator / SLF Medication Monitor
Basic Function: Monitor SLF patient self-administration of medication
Reports To: Director of Nursing
Hours: Fulltime/Hours to be determined based on need
Salary: Hourly rate position based on education and experience.
Evaluations: 90 days and yearly
Education: LPN or bachelor’s degree in health-related field preferred. Minimum of 1-year healthcare experience with preference of 2 years’ experience.
Experience: As this position will work semi-independently: must have 3 years of medication administration experience.
Responsibilities:
1. Monitor patient Medication pick-up and dispensation of medication.
2. Patient Medication education
3. Reviewing medications with new IOP, SLF patients.
4. Always maintains patient safety.
5. Help provide and maintain therapeutic medium for patients.
6. Actively participates in staff meetings.
7. Provides competent nursing care of all patients within the boundaries of licensure.
8. Complete admission process including nursing assessment upon admission.
9. Monitors vital signs and mental status as needed.
10. Assess physical complaints and problems making any referrals to Medical Director and/or Emergency Room.
11. Supervise self-administration of medications.
12. Obtain urine samples for drug screens as ordered.
13. Inventory supplies emergency box, first aid kit and medications and notify nursing coordinator/manager of needs.
14. Accurate implementation of physician orders.
15. Patient teachings as indicated by special needs, demonstrated lack of knowledge and assessments.
16. Thorough and accurate documentation of patient care, procedures, complications, and progress in medical record using the problem-oriented record system.
17. Orient patients to program and phases of treatment.
18. Develops, implements, and maintains current nursing care plans for assigned patients
19. Assist the medical provider in the coordination of patient care involving MAT services.
20. Perform prior authorizations with third party payers for medication assisted therapies used in the treatment of substance use disorders.
21. Know and abide by the general provisions of CFR 42 Part 2 Confidentiality of Drug and Alcohol Abuse Patient Records, and Health Insurance Portability and Accountability Act. This position has unrestricted access to patient identifying and patient health information.
22. Participates in discharge planning for MAT services.
23. Facilitates follow up on the status of all commercial insurance for MAT and on MAT that is in the process of approval.
24. Coordinates injectable MAT preparations with the medical team.
25. Responsible for shipments and storage of injectable forms of MAT and the associated management of co-pays and deductible payments.
26. Working with the medical provider and the discharge planning team to arrange the seamless transition of MAT care from the inpatient setting, into the outpatient setting.
27. Works with specialty pharmacies in the ordering of injectable forms of MAT.
28. Process all patients on the daily list. This will include submitting
MAT treatment plans to insurance companies for approval.
29. Other duties as assigned.
