Location: Independence, OH We are seeking a part-time or full-time qualified nutrition support associate to join our team in an on-site/hybrid position. Our Nutritional Support Coordinators are either in-office or remotely based to provide care to patients across the United States. The ideal teammate would be: An extension of the provider to help close the gaps in your patient’s care. As part of our amazing team, you will build a rapport with your patients through telehealth calls as you support them through services such as nutritional counseling to assess their dietary needs, health goals, and lifestyle factors. You will work with patients and their providers to provide educational guidance on developing meal ideas, developing and maintaining healthy eating habits, and providing support in maintaining weight related health goals and wellness needs. In addition, you will advocate for their nutritional needs by utilizing active listening skills, investigating patient concerns, documenting appropriately, and communicating professionally with the patient’s care circle to advocate a resolution to those needs. The ideal teammate would be able to:
Manage and support patients' nutritional needs via phone conversations
Demonstrate fluency in multiple languages to facilitate clear and accurate communication with a diverse patient base.
Ensure that daily production goals and quality metrics are met within standards set forth by Cosán Group
Complete individualized patient nutritional care plans and perform coordination services via our software platform
Gather pertinent information related to patients’ nutritional needs and dietary needs
Monitor and follow-up with nutrition related aspects of various chronic conditions and medications
Maintain frequent contact with patients to provide nutritional support, coordination of care related to nutrition and manage compliance with the care management programs to increase positive outcomes
Collaborate with Cosán clinical team members, providers, and other members of the patient’s healthcare team to endure a coordinated and cohesive approach to patient care
Document all communications with patients, their caregivers, and their providers accurately
Communicate to and from facility or practice care clinicians regarding patient needs and/or life-threatening episodes and to ensure comprehensive care plans are complete and accurate
Adhere to documentation protocols and best practices for daily workflow, escalation of client issues, comprehensive care plans, and internal communications
Exhibit excellent customer service skills demonstrated by positive feedback from customers and patients
Demonstrate an ability to develop and build trusting relationships with patients and partner practices with a true passion for caring for patients and guiding them in their health journeys
Have an ability to stay flexible and quickly adapt to new processes, software changes, and priorities with a positive attitude and a willingness to learn new skills on a continuous basis
Must be an empathetic and active listener with the ability to thrive both as a team member and independently in a fast-paced environment
Required Qualifications
See license/certification/education section
Ability to translate medical information, instructions, and documents as needed to ensure comprehensive understanding
Previous adult in-home, in-facility or remote chronic conditions care management addressing nutritional needs
Previous experience in nutritional care management preferred
Demonstration of fundamental nutrition knowledge and nutritional education and coaching skills
Work Requirements for Full-Time Hires : To ensure an ability to work effectively remotely (while at home), the following is required:
A high-speed, reliable broadband internet connection
Cosán will provide computer and telephonic equipment appropriate to meet the job requirements
A dedicated space isolated from interruptions to ensure you, your patients and office staff have continuous privacy to accomplish daily activities and protect member confidential information (per compliance with HIPAA privacy laws)
Work Specifications
Part-time hires are in-office exclusively
Full-time hires are allowed a hybrid schedule with a 2-day per week minimum in-office expectation following a 3–12-week training period depending on employee success meeting performance expectations
