Your duties will include:
- Performing the ANSA assessment
- Providing individualized placement and support for supported employment
- Providing individuals with a mental illness with community-based supports and individualized activities that promote recovery, self-determination, self-advocacy, well-being and independence through a relationship that supports a person’s ability to promote their own recovery
- Assessing the individual’s physical and mental health care and support needs, strengths and preferences, available service and housing options, and when unpaid caregivers will be relied upon to implement the Person-Centered Plan
- Coordinating all services received by an individual and assisting the individual in gaining access to needed Medicaid State Plan and 1915(i) services, as well as medical, social, educational, and other resources, regardless of funding source.
- Coordinating activities that help individuals gain access to needed health (physical and behavioral health) services
- Participating in the care planning process as a member of the trans-disciplinary team
- Responding to and assessing emergency situations and incidents and assuring that appropriate actions are taken to protect the health, welfare, wellness, and safety of individuals
Qualifications:
- Bachelor’s degree in social work, counseling, psychology, or similar field;
- Minimum of 3 years post degree experience working with individuals with severe and persistent mental illness (SPMI);
- A good working knowledge and understanding of issues related to the mental and behavioral health population.
- Knowledge of the individual, family, social and the individuals needs functioning with mental health issues. .
- Good customer service skills.
- Good interpersonal communication skills.
- Good organizational and teamwork skills.
- A valid driver’s license and reliable transportation for travel
- Proficient computer skills including the Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed.
- Ability to travel within the state of Ohio for training.
