Overview

Supervisor Care Coordinator Services, Clinical Access 

Full Time, 80 Hours Per Pay Period, Day Shift

Position Summary:

Responsible for Supervision of Healthcare Outreach Specialists in providing care monitoring and in-person support services to assigned consumers. Position is responsible for training of staff, scheduling and assuring follow up for service participants into services both within the healthcare service delivery system and the general community. Assists the Manager of Care Coordination in assuring the development of comprehensive care plans for TN Health Link members. Responsible for gathering data as needed to assure the quality and effectiveness of the program.

Responsibilities

  • Set goals for performance and deadlines in ways that comply with company’s plans and vision and communicate them to subordinates.
  • Organize workflow and ensure that employees understand their duties or delegated tasks.
  • Monitor employee productivity and provide constructive feedback and coaching.
  • Receive complaints and resolve problems.
  • Maintain timekeeping and personnel records.
  • Pass on information from upper management to employees and vice versa.
  • Ensure adherence to legal and company policies and procedures and undertake disciplinary actions if the need arises.
  • Prepare and submit performance reports.
  • Decide on reward and promotion based on performance.
  • Hire and train new employees.
  • Contribute to development of comprehensive care plan for designated service participants.
  • Promotes excellent customer service and leads customer service recovery efforts when the need arises.
  • Assess and assist service participants during times of crisis in order to facilitate re-stabilization.
  • Provide on-going linkage, monitoring, planning and evaluation, and advocacy for team consumers.
  • Participates in transitional care task force
  • Assess eligibility and assist consumers in obtaining financial entitlement, such as SSI, SSDI, and all other programs they may be eligible to receive.
  • Local travel required.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.

Qualifications

Minimum Education: Bachelor’s Degree in a health-related field of counseling, psychology, or social work preferred

Minimum Experience: One (1) year relevant experience if bachelor’s degree is not in counseling, psychology, or social work; otherwise, none.

Licensure Requirement: Employee must have a valid Tennessee driver’s license Class D and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure.
