Covenant House New York is Hiring a Case Manager, Bainbridge (54267) Near Bronx, NY
PURPOSE OF POSITIONThe Older Females Program Case Manager provides supportive services to 20 single young women ages 21-24 at a shelter site located in the Bronx funded by DYCD. Reporting to the Program Coordinator of the shelter site, the Case Manager is an energetic, creative, and diligent member of a team that includes resident advisors (who also have cooking responsibilities), and is a diligent member of a team that includes case managers and a specialty of support staff. The Case Manager will provide comprehensive case management services to the youth who are on their caseload including: creation of an individual service plan, coordination of services, individual counseling, advocacy, monitoring of the youths efforts and challenges in completing goals, and celebrating goal achievement. Duties And Responsibilities (Responsibilities to include but not limited to the following)
Case Management
Provides integrated counseling and case management services that effectively engage young adults and promote personal growth and wellness, housing stability and accomplishment of service plan goals on an ongoing basis.
Collaborates with youth to assess strengths and challenges and develop an individual service plan that reflects the youths progress and stated goals.
Meets with youth regularly to determine progress with goals identified in the ISP.
Advocates on behalf of youth with outside service providers and other collateral services.
Creates and maintains confidential records, charts and logs for youth documenting all relevant information from individual and group interventions as well as with collateral services.
Ensures that youths files are compliant with all regulatory and other requirements.
Completes incident reports per agency policies.
De-escalates crises; attends to emergencies following agency protocol, as needed.
Team Participation
Works as a collaborative member of the team ensuring all of the young women have access to high quality services and are living in a safe and compassionate community.
Participates in regular case conferences. Ensures team members are fully apprised of important information.
Actively participates in staff team meetings, individual supervision meetings, and meetings with the young women.
Assists with audit preparation and dry and live audits of the program.
Bachelor's degree required, preferably in a relevant field.
One (1) year of experience in a residential setting working with adolescents is preferred.
Demonstrated knowledge of and experience working directly with young people through approaches that emphasize resiliency, strength-based youth development, and trauma-informed care required.
Knowledge/Skills
Strong multi-tasking skills with demonstrated ability to manage multiple projects at once required.
Excellent verbal, written and problem-solving abilities and an upbeat demeanor required.
A strong focus on child protection and ensuring the safety of residents is required.
Experience working with diverse groups required.
Overall flexibility in work schedule required, including weekends, evenings and holidays if needed.
Experience utilizing evidence-based and other social service models preferred.
