CPC Integrated Health is Hiring a Care Manager, Outpatient (Full-Time, Red Bank) Near Red Bank, NJ
POSITION SUMMARY: Care Managers will provide services which include clinical documentation, crisis intervention and outreach services. Care Managers work in collaboration with the consumer and the treatment team in order to maximize the consumers' quality of life. The Care Manager coordinates all needed client services with other providers and significant others in the community.

- BA Degree in Mental health or related field.
- Ability to communicate effectively and clearly in written and spoken English.
- BA Degree in Mental Health or related field with mental health experience and 3 years experience or Master's Degree in related field.
- Completes assessments and/or assists other staff in completing assessments in a timely and comprehensive manner as required by CPC and regulatory agency policies.
- Responsible for timely and accurate completion of all documentation and paperwork as per Agency policies and procedures.
- Provides services to clients and their families. Services include education about their illness and medications; assistance with day-to-day problems; referrals, advocacy; and crisis intervention.
- Responsible for reporting to supervisor all client-related problems.
- Provides linkages to natural community supports to access needed services in order to maximize independent living.
- Coordinates with other agencies or professionals to provide services to the consumer.
- Responsible for knowledge of community resources especially emergency services.
- Schedules will be flexible depending upon program and client needs.
- Participates in overall coordination and planning of services
- Works cooperatively with other staff and supervisor.
- Seeks supervision as needed in a professional and competent fashion.
