CPC Integrated Health is Hiring a Program Supervisor, ICMS (Full-Time, Eatontown) Near Monmouth, NJ

POSITION SUMMARY:

The Program Supervisor assists the Sr. Coordinator with managing the day-to-day activities and the staff of the ICMS Program. The Supervisor also works in collaboration with clients in planning, implementing, and evaluating service provisions to maximize clients' independence and success in the community. The Program Supervisor completes initial assessments; prepares a treatment plan; provides case management services; documents ongoing treatment and discharge planning. This is a community based position.

MINIMAL QUALIFICATIONS:

  • Must minimally possess an active LSW or LAC. LCSW or LPC preferred.
  • Must have at least 1 year post degree experience providing treatment services to adults.
  • Experience with evidence-based practices.
  • Experience working in the community.

MAJOR RESPONSIBILITIES/PERFORMANCE OBJECTIVES:

  • Assist the Sr. Coordinator with the day-to-day operations of the program and supervising the ICMS staff.
  • Complete Biopsychosocial Assessment within the time frame established for the program and formulate treatment plan. Utilize DSMV to determine diagnosis and level of care. Utilize a holistic approach to complete the biopsychosocial assessment that addresses all life domains in clear and understandable language. Include client and family in the treatment planning process.
  • Complete Risk Assessment at intake, notify supervisor if risk level changes, and monitor High Risk clients weekly.
  • Complete individualized treatment plans utilizing evidence-based treatment approaches such as Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), Stages of Change and Relapse Prevention in work with clients. Utilize a holistic approach towards developing treatment plans.
  • Provide direct services to clients and their families (i.e., symptom management; assistance with structuring and performing basic daily living skills including money management; development and support of social and recreational activities; client psycho education and support family and significant others education and support; client advocacy; crisis intervention; securing and maintaining community integration including addressing housing needs and financial resources; transportation for clients in agency vehicles with adherence to agency policies and procedures; and educates and supports community regarding needs of person with mental illness.
  • Coordinate with client's family and other agencies that provide services to client.
  • Participate in an interdisciplinary team process to ensure the provision of services and/or linkage with requisite services to client.
  • Record, report and monitor service delivery as per program contract requirements.
