• 36 hours per week, 3x12N 1900-0730 Possible Floating to support staffing within Med/Surg Every other Weekend Every other Holiday No On Call requirements BLS required EHR-EPIC ONLY contracts 75 mile radius considered

Roles & Responsibilities

• Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.

Pay: $49.10 - $51.10 per hour
