Registered Nurse (RN) Case Manager Requirements
- Full-time, 13-week contract position with a local Public Health Department.
- In this role, you will be assigned a caseload of clients who have a variety of medical needs, mental health issues, and/or transitional support needs that fall between the long-term care or acute care facility and the home. The RN will work closely with the client's healthcare team to assure that the client remains safely in their home.
- This is an office-based role in Oakland, CA, with virtual client visits.
- Monday-Friday, 8am-4:30pm 40 hours per week. some weekends as needed.
- Requirements: CA RN licensure, AHA BLS
- Requires 2 years experience as an RN, case management
