Palliative Care Coordinator Qualifications:
- Must possess a current, unencumbered LPN License
- Experience in scheduling in a healthcare organization or unit
- Receive and promptly process palliative care referrals and/or inquiries for services provided by Crossroads
- Work in cooperation with the call center for incoming or received referrals
- Adept knowledge of Palliative Care program and ability to explain to the fullest extent palliative care utilization and services available
- Maintain accurate and detailed records of all information calls and referrals
- Schedule and assign as appropriate all palliative patients with designated Palliative Care Nurse Practitioner(s)
- Additional duties as assigned
- Be part of a team willing to grow, listen, be heard, and be challenged
