Registered Nurse Case Managers deliver direct care to patients in their homes, whether it's at home or in a care facility. This trust allows us to serve as the central point of communication within our interdisciplinary team of holistic care providers.

• 6 months minimum prior skilled nursing experience
• An active unencumbered state Registered Nurse license
• Current driver's license and auto liability coverage
• Interest and Ability in providing patient care in homes and facilities

Registered Nurse Benefits & Schedule:
