Job SummaryWe are seeking a highly organized and compassionate Day Habilitation Service Coordinator to join our team. The successful candidate will be responsible for coordinating and overseeing the day-to-day operations of our day habilitation services. The Day Habilitation Service Coordinator will work closely with clients, families, and staff to ensure that all services are delivered in a safe, effective, and timely manner.

Job SummaryWe are seeking a highly organized and compassionate Day Habilitation Service Coordinator to join our team. The successful candidate will be responsible for coordinating and overseeing the day-to-day operations of our day habilitation services. The Day Habilitation Service Coordinator will work closely with clients, families, and staff to ensure that all services are delivered in a safe, effective, and timely manner.
Responsibilities
- Develop, coordinate, and monitor individual program plans and behavior support programs, as appropriate
- Implement, coordinate, and facilitate client staffing’s on a regular basis, and other interdisciplinary meetings as needed
- Maintain consistent communication with interdisciplinary team members as pertaining to assigned caseload
- Maintain current and accurate records for assigned caseload files
- Advocate for needed services and programs for assigned caseload
- Be knowledgeable regarding current methods and techniques of behavioral programming
- Participate in the Program Quality Review process to ensure accuracy and quality of program files
- May participate in medication manager training in order to administer daily medications to individuals requiring assistance
- Communicate any workplace needs by following the correct chain of command
- Attend Agency meetings, trainings, and participate in committees as directed by the DH Manager
- Audit Day Hab daily documentation for accuracy and compliancy.
- Bachelor's degree in social work, psychology, or a related field preferred.
- Minimum of 1 years of experience in a similar role working with persons with disabilities
- Valid driver's license and pass background checks showing no evidence of founded abuse, or criminal record
- Knowledge of state and federal regulations related to day habilitation services
- Excellent organizational and time management skills
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient demeanor
- Ability to maintain confidentiality and professionalism at all times
