We are currently searching for a dynamic individual to become the Intensive Case Manager for the unit. The Intensive Care Manager assists the patient and the family in accessing different programs facilitates coordination between the systems involved and links them to community resources.
All positions require pre-employment drug screening and background checks.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
Meet with patients regularly to help them access additional programs and services beyond drug and alcohol treatment.
Refer participants to appropriate assignments according to established procedures.
Ensure that participants understand their obligation to participate in the Intensive Case Management Program and the requirements.
Contact participants on the business day before their appointments to remind them of these appointments.
Advise participants that ICM services are available until employment is found, and they are required to participate in another program or until their time expires.
At each visit, remind them about time limits and advise them of the number of days they have left on aid.
Explore reasons why the participant has been unsuccessful in obtaining employment, and if necessary, make mental health, domestic violence, substance abuse, or medical evaluation referrals according to established procedures.
Reinforce/enhance activities like dress for success, resumes, applications, etc.
Review, revise, and update resumes and related documents to ensure they are accurate, timely, and effective.
Have the participants complete a “test” job application to ensure they can fill it out appropriately.
Provide referrals to job fairs.
QUALIFICATIONS:
Education:
High School Diploma or GED required.
A Certified Addiction Technician (CAT) must be received within 18 months from the hire date.
Experience:
One (1) to two (2) years of experience providing effective case management to behavioral health patients.
Other:
Computer literate, with basic knowledge of Microsoft Office and a high initiative in keeping current with technological changes.
Ability to prioritize workload and activities of self and complete tasks promptly and efficiently.
Ability to set boundaries, resolve conflict, and de-escalate issues.
Dependable, able to work under pressure, receptive to change, willing to learn, and cooperative approach to problem-solving.
Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from various ethnic, socioeconomic, and cultural backgrounds, and good diplomatic skills.
Team player.
Excellent attention to detail.
Excellent communication skills in English (written & verbal).
PHYSICAL DEMANDS:
The employee must occasionally lift and move up to 30 pounds.
WORK ENVIRONMENT:
Work is typically performed in a standard office setting and waking. The noise level is usually moderate.
