Crown Home Health is looking for an experienced LVN Case Manager to manage patient's care in home health settings. We are a home health provider that services all throughout Southern California.

Job Description Summary
Responsible for coordinating home health services for managed care patients, which includes establishing and maintaining the patient’s plan of care, establishing and maintaining positive relationships with home health patients.
Essential Job Functions/Responsibilities
1. Coordinates home health services with the services of other care providers, community care providers and coordinates community resources.
2. Maintains confidentiality and safeguards all patient information.
3. Responds to questions of employees, patients, physicians, and other allied healthcare providers about the home health services being provided.
4. Establish a cooperative relationship and communicates effectively and professionally with physicians. Participates in patient care conferences
6. Documents and communicates to all patient/family the home health services provided as required by the company policies and applicable regulations.
7. Prepares progress notes, in reference to client care coordination. Participates in discharge planning.
9. Provides quality assurance through careful review of both technical and clinical content of the charts to ensure accuracy, completeness, and consistency
10. Responsible for analyzing audit data and initiates interventions for quality improvement studies and activities
11. Other functions assigned by the HH Administrator.

Position Qualifications
1. Current LVN License
2. At least six (6) months of experience in home health care coordination.
3. Complies with acceptable professional standards and practice.
6. Demonstrates good communication, negotiation, and public relations skills.
7. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.

Pay: $31.00 - $35.00 per hour
