Position Summary: 

• Coordinates case assignments and visit schedule for all home health field staff .
• Performs clerical duties.
• Works in conjuction with the Intake team to schedule all patient visits
• Communicates with all patients to schedule all new admissions
• Works as a self-starter. Recognizes and performs tasks to be completed and/or services to be provided. Completes work independently.
• Maintains up to date knowledge in the use of the home health clinical software.
• Utilizes workstation for patient care documentation, document communication and email as appropriate. Maintains computer security.
• Participates in educational programs and inservices to enhance skills and development 
• Willingly and readily, assists all internal and external customers as needed, looking beyond the individual scope of service.
• Interacts with patients, visitors, staff and other customers in a courteous, professional and service oriented manner. Communicates in a positive, helpful and friendly manner at all times, using appropriate body language, eye contact and tone of voice
• Home Health experience preferred
