Description

Summary:

• Reviews home care and hospice SN, AIDE, MSW daily notes for accuracy as needed
• Analyzes data to present quarterly QAPI data for home care and hospice
• Provides feedback to home care and hospice supervisors for needed QA improvements
• Attends education classes, IDT, and case conference as needed
• Assist Hospice and home care teams as needed in day-to-day operations
• Provides one-on-one education/QA review with appropriate clinicians
• Demonstrates and encourages team behavior and exceptional patient/guest experiences.
• Upholds and promotes patient safety and quality.

Qualifications

Minimum Knowledge, Skills, Experience Required

Education:

license in practical nursing or associate degree in nursing required

Experience:

Minimal 2 years’ experience as home care field nurse required. Preferred experience in hospice. Preferred hospital Med/Surg; ER or Critical Care experience.

Additional Skills / Abilities:

Leadership skills; great communication skills; critical thinking skills; knowledge in use of Microsoft and Excel; flexibility and ability to multitask and prioritize. Must be organized and driven to complete tasks by a deadline.
