CVP is Hiring a Registered Nurse Care Coordinator/Case Manager Near Charlotte, NC

Job DescriptionCVP is seeking a Registered Nurse Care Coordinator/Case Manager to join our team in providing our Nation’s Veteran’s with Hematology and Oncology specialized telehealth services for the Durham VA Healthcare System in Durham, NC. Working hours are Monday-Friday, 8:00-4:30 pm EST.
Responsibilities:
  • Ensure that all incoming consults are triaged within two business days of receipt.
  • Contact patients with negative results within three business days of when they are returned.
  • Update patient tracker with relevant patient information with minimal errors.
  • Ensure that consults to any other service are input and read for signature.
Requirements:
  • Ability to pass a required level of security clearance (US Citizenship Required)
  • Associate Degree or Bachelor of Science in Nursing from an accredited program, college, or university and hold a current license to practice as a Registered Nurse in any State; a Bachelor’s degree is preferred
  • Experience with RN activities in providing hematology/oncology care for a minimum of 2 years
  • Telehealth Care Coordination/Case Manager experience required
  • Specialization in Genetics
  • Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Certified
  • Familiarity with medical terminology, hospital terminology and/or clinics.
  • Technically proficient in the skills necessary to fulfill the government's requirements; will be provided training by the SFVAHS on appointment management, VA policy and procedures.
