Job Summary: This position is primarily responsible for providing mental health/substance abuse consultation, assessment, brief intervention and referral to individuals accessing medical services. This position will work collaboratively with Daily Planet Health Services’ medical, behavioral health, and case management teams to provide comprehensive services to Daily Planet’s patients. Participates in providing psychosocial and other services to patients that qualify under the Ryan White Consortium. Participates in all safety programs which may include assignment to an emergency response team.

Job Requirements: Masters’ degree in Social Work or relevant field required. LCSW, LPC or related licensure preferred. Two (2) to four (4) years of providing therapeutic behavioral health services experience required. Knowledge in medical, mental health and substance abuse issues required. Experience working with the homeless and uninsured preferred. Bilingual (Spanish/English) preferred. Skills include excellent oral and written communications including telephone, organizational, and personal computer including word processing and Microsoft office.
