The position involves coordination and collaboration with families, schools, law enforcement, court, and service providers.· A Bachelor's degree in Social Work, Psychology, or Sociology
AND
· One year of full-time professional Social Work experience OR Two years of paraprofessional experience working in a human service related agency
AND
· Valid Driver's License
PREFERRED EXPERIENCE BEYOND MINIMUM QUALIFICATIONS:
· A minimum of one year of experience in children and family services or children's mental health case management
· Experience working with diverse cultures
· Previous experience in a county social service agency
· Licensure as a Mental Health Professional
· Experience in Behavioral Analysis and the development of Positive Behavioral Support Plans

1. Assesses individual strengths, needs, and family dynamics.
2. Works with children and families in a variety of situations, including special education, truancy, runaways, chemical dependency, abuse, neglect, and general family conflicts.
3. Refers clients to community providers for services.
4. Monitors and verifies services received by individuals and families and develops reports and resources as needed.
5. Completes administrative tasks involving case plans and other legally mandated or best practice based procedures.
6. Establishes positive relationships with parents and children and works collaboratively with community agencies.
7. Other duties as related or assigned.
8. Demonstrates a commitment to support cultural diversity County wide, actively supports the County's diversity initiatives, and works to address the needs of diverse and underserved communities.ESSENTIAL FUNCTIONS: 1-8· Knowledge of human development, family dynamics and systems, including crisis management and cultural competency.
· Knowledge of community resources and funding sources.
· Knowledge of medical diagnoses, medications, and basic medical terminology, as well as medical insurance processes and procedures.
· Knowledge of statutes, rules, and policies pertaining to social work and the ability to research the same.
· Comprehensive knowledge of computer software and information systems.
· Interviewing, negotiation, and listening skills.
· Ability to communicate effectively both orally and in writing.
· Demonstrates an understanding of and respect for the diversity of customers, co-workers, and supervisors, including individuals with a disability or whose first language may be other than English.WORK ENVIRONMENT: Work is performed remotely, in a County office building and on client home visits. Some travel is required. Lifting requirement of up to 20 pounds on an occasional basis. Exposure to hazardous physical conditions such as mechanical parts, electrical currents and vibration; atmospheric conditions such as fumes, odors, dusts, gases and poor ventilation; hazardous materials such as chemicals, blood, and other body fluids; inadequate lighting; work space restricting movement; and intense noise may occur on visits to client's homes. Equipment used may include, but is not limited to, computer, printer, telephone, calculator, and copy and fax machines.
