Care Coordinator: Crossroads Continuum Program

POSITION DETAILS:

• Coordinating screening, treatment, and supportive services to promote recovery from substance use disorders.

• Providing referral, advocacy, and case coordination services with goal of linking individuals to appropriate treatment levels of care.

• Linking responsibilities, accessing/utilizing other services, contacting partner agencies, and coordinating delivery of services.

• Assuming an advocacy role for clients, including advocating for admission to other services.

• Functioning as part of a multi-disciplinary team.

• Assisting Supervisor to manage wait lists and referrals.

• Supporting all Clinical Staff in accelerating timeliness of service provision.

• Participating in treatment team meetings and assisting with client interventions to support recovery and linking with treatment.

• Providing Tele Health wellness calls and coordinating internal/external services.

• Updating outside referrals and thorough electronic medical records completion.

• Developing strong community partnerships and collaborations to engage referrals and transitions in care.

TO BECOME A MEMBER OF OUR CROSSROADS CLINICAL TEAM:

• Bachelor's Degree from a regionally accredited college or university in psychology, social work,

behavioral science, or related area of study OR minimum of 5 years of related experience = Required

• Spanish Bilingual = Preferred But Not Required

• Experience working with substance use disorders, mental health, and co-occurring disorders = Required

• Knowledge of recovery and relapse prevention = Strongly Preferred

• Exceptional and intuitive track record in assessing client needs = Required

• Efficient, empathetic, and evocative communication skills = Required

• Demonstrated and experienced-based client service abilities = Required

• Valid FL Driver's License and clean driving record = Required

• Accurate reporting and record keeping skills = Required

• Staying compliant on all trainings and immunizations = Required

• Ability to pass a Level II DCF/AHCA Fingerprinting, Drug Screen & Local Criminal Check = Required
