David’s Loft Clinical Programs (DLCP), an Integrated Behavioral Health Organization, is seeking dedicated and dynamic LPN to fill our full-time role. Under the direction of the Medical Director, and the Nurse Manager, the LPN is a member of a multidisciplinary care team. The Primary Care Office includes: Family NPs, Psych NPs, Licensed Clinical Professional Counselors, Licensed Social Workers, Certified Substance Use Counselors, Certified Peer Recovery Specialists, Medical Assistants, and lay staff all working as a team to support individuals with co-occurring substance use and mental health disorders. This role is best fit for someone who is a critical thinker, self-starter, and has great compassion for those suffering from substance use disorders. 

DLCP is a fast-paced environment treating individuals with chronic somatic and behavioral disorders. This role is a Monday-Friday position with weekends off.

Shift: Mon, Tues, Thurs, Fri (9:00 AM to 5:00 PM) and Wed (11:00 AM to 7:00 PM) 

Responsibilities:

• Taking and monitoring patient vital signs
• Assisting patients with answering any questions they may have pertaining to their treatment within an LPNs scope of practice
• Administering medication
• Communicating changes in patient behavior or health status to Nurses, Physicians, and Clinical Social Workers, and Program Managers
• Maintaining accurate patient records 
• Collecting specimen samples when needed
• Taking an active part in patient care planning
• Preparing patients for transfers or discharge
• Providing patient education as needed
• Managing emergency medical situations as they arise with urgency and accuracy
• Other duties as assigned by supervisor/clinical treatment team
