The LPN is responsible for providing administrative and clinical support to ensure efficient operation of our treatment program within an integrated behavioral health organization. The LPN will support the Prescriber, Behavioral Health Clinicians, and patients through a variety of tasks related to patient care management, organization, med pass and communication. Selected candidate will work in our Fayette Street location.
The LPN will possess a high school diploma or equivalent. Certification as a Licensed Practical Nurse required.
Salary Range: $18 - $23 per hour. Based on the accumulative certifications possessed.
Available Shifts: 8 AM - 8 PM (Weekends Only) | Non- Exempt
Areas of Responsibility:
1. Provide Administrative support to the Prescriber primarily and Behavior Health Clinicians on an as needed basis.
2. Schedule and maintain appointment Calendars for the Prescriber.
3. Take patient vitals prior to seeing the Prescriber.
4. Communicate no show information to appropriate parties.
5. Assist Clinicians with their calendars on a as needed basis.
6. Corresponding with medical practitioners to ensure appropriate medical care is provided.
7. File Somatic Health medical documentation in the client’s electronic and paper record.
8. When notified, retrieve discharge documentation from individuals visiting an emergency room, urgent care, or inpatient hospital stay for behavioral health treatment.
9. Make follow-up appointments as indicated by visit and or discharge documentation.
10. Request medical records from providers.
11. Administers medication as needed.
12. Escort patients to see practitioner.
13. Assist with Records Maintenance.
14. Assist Somatic Health Clinicians with supplies etc.
15. Cover’s Front Office in emergencies
16. Other duties as assigned within the scope of your role
