Daybreak Independent Services is Hiring a Quality Assurance Coordinator Near Bronx, NY

Job Title: Quality Assurance Coordinator
Reports to: Chief Compliance Officer (CCO)
Location: Bronx, NY
Job Classification: (Full-time)
FLSA: Exempt
Salary Range: $60,000 - $62,800 annually
The primary mission of DBIS is to provide quality service to our individuals in a professional and caring manner.
Education & Experience:
  • Bachelor's Degree in social work preferred special education or related field.
  • One year of direct experience working in Developmental Disabilities adult service programs (day and residential) is preferred.(OPWDD) experience is preferred
  • Excellent attention to detail
  • Ability to get along well with co-workers and many levels of administrative, management, and direct support staff
  • Capable of working under pressure and deadlines
  • Responsive to supervision
  • Flexibility and a strong work ethic; willing to take on additional tasks and work beyond regular work hours when required
  • Proficiency in Microsoft Office Suite, including Word, Excel, PowerPoint, and Access
  • Driver License.
Summary of Primary Responsibilities and Essential Functions of Position:
  • QA Coordinator will work directly under the CCO
  • Oversee QA Specialist on their daily functions with audits, billing & quality review reports and provide training & support as needed.
  • Responsible for maintaining and tracking appropriate documents, records, statistics, reports, evaluations, and other required documents.
  • Monitor quality of services at each site and generate reports based on the observation by teaming up with QA, Programs, Clinical & QIDP team.
  • Assist CCO during HIPAA and billing audits, and participate in unannounced program observations (such as fire drills, staff interaction/observation, physical plant walkthrough, etc.) as per DOH/OPWDD/OMIG regulations. Help in developing corrective action plans and follow up as needed.
  • Led the internal audits and reviews to ensure the documents complied with OMIG, OPWDD, and DBIS's standards.
  • Ensures that all necessary documents are stored electronically per the standards of DBIS.
  • Reviews QA Specialist project reports and seeks corrective action as needed.
  • In the absence of the QA Investigator monitors daily incidents from all sites. Review and maintain records per Justice Center and OPWDD regulatory guidelines and contact programs as needed. Monitor IRMA, agency incident chat, and agency incident mailbox. Report concerns to the CCO.
  • Monitors agency incident mailbox and communicates information effectively.
  • Assists CCO with the HRC/IRC review meeting and conducts part 624/625 incident reporting and other QA training as needed and when requested.
  • Monitor and observe monthly fire drills at all sites. Keep records, review, and address concerns for corrective action as needed. Send copies to the CCO.
  • Performs daily examinations of program functions and system checks and provides systematic follow-up.
  • Completes all other required documentation by required timeframes adhering to the agency protocols and OPWDD/OMIG rules & regulations.
  • Creates and runs reports to identify trends utilizing agency-approved software. Reports findings and recommendations to the CCO.
  • Use electronic databases to perform day-day duties such as EVERO, CHOICES, IRMA, Paycor, etc.
  • Conducts site visits in all agency program locations to assist with audit preparedness and relationship building.
  • When required, communicate effectively between DBIS programs and external agencies such as MHLS, OPWDD IMU, and Justice Center.
  • Participate in committee meetings and training when requested.
  • Travel between the worksites when requested for job-related duties and as needed.
  • Conduct Investigation as needed.
  • Perform other related duties as requested.
