What are the responsibilities and job description for the Neuro Program Coordinator position at Deaconess Hospital, Inc.?
Job Duties include the following, other duties may be assigned:
Facilitates the overall multi-disciplinary Stroke Operational and Peer Review Teams - assessing, developing,
planning, implementing and evaluating the overall stroke program.
Positively impacts the quality of patient care by functioning as a practitioner, consultant, researcher, educator
and change agent throughout the Department of Nursing and other hospital areas for the Stroke Program.
Any issues are reported to the Stroke Medical Director for follow up as needed.
Reviews departmental nursing education plans and provides recommendations for required education and
skills development.
Identifies areas for improvement in the stroke care areas including reviewing quality of care issues with
managers in regard to stroke patients.
Reviews departmental nursing education plans and provides recommendations for required education and
skills development and is a mentor to stroke center nursing staff.
Participates in the process monitoring performance improvement indicators in the stroke patient.
Coordinates action planning and documentation between the Stroke Program and the hospital-wide PI
Program.
Responsible for data processing, analyzing, and reporting variances to the Stroke Operational and Peer Review
Teams, the Medicine Core Care Team and the Quality Team as needed.
Facilitates efficient, effective, quality care for the Neuro medical population. Collaborates with the
interdisciplinary team to enhance customer satisfaction, and to maximize patient outcomes.
Gathers and analyzes data for benchmarking and submission for Center of Excellence recognition.
Serves as a liaison between physicians, patients and clinical staff.
Coordinates “Get With the Guidelines” stroke data base to ensure data entry is accurate and validates any
information being reported. Collaborates with the Quality Analyst responsible for stroke measures to ensure
QI reporting is meeting needs for regulatory requirements; I.e. ACHC and CMS.
