JOB TITLE: Reentry Case Manager

REPORTS TO: Delphi Rise Monroe County Reentry Coordinator

PAY RANGE: $25.00-$26.50 Per Hour (depending on education, experience and qualifications)

Pay Differential: Additional $2.00 Per Hour for bilingual proficiency in Spanish.

ABOUT DELPHI RISE:

Delphi Rise is a small but impactful community-based, non-profit behavioral health organization that focuses on supporting individuals with substance use addiction and other complex needs. Since 1936, we have been offering our community an array of services including Outpatient Treatment; school-based prevention, education and counseling; prison Reentry Services; HUD Rapid Re-Housing supports; and Health Homes Care Management services. We also operate an Open Access Clinic, providing on-demand substance abuse evaluations and referrals. In fact, we are the only 24/7 Open Access Provider in Monroe County!

SUMMARY:

This position is responsible for providing person-centered case management services to clients in the Monroe County Reentry Program, serving people released from state prison. Our goal is to enhance public safety and decrease recidivism by helping our clients become stable in the community and engage in productive, healthy behaviors.

JOB RESPONSIBILITIES:

· Provide services to a caseload of 30 to 40 clients, supporting clients in obtaining benefits, employment, treatment, housing, childcare, healthcare, identification paperwork, and other key supports.

· Coordinate client services with NYS Dept. of Corrections and Community Supervision (Parole), Monroe County Probation, the Court system, or other collaborative partners.

· Maintain accurate and timely client records and case notes.

· Create a comprehensive service plan for each client, providing necessary resources and support for successful completion.

· Provide or arrange for appropriate support groups, including Cognitive Behavioral Therapy, parenting, domestic violence, and anger management.

· Meet with clients when necessary and relevant to success.

· Active participation in Monroe County Reentry Task Force meetings.

· Perform other tasks as assigned by management.

DESIRED SKILLS/ABILITIES:

· Able to successfully engage clients, including those who may have serious, persistent mental health issues and criminal justice histories

· Familiarity with resources in the community that meet a wide variety of client needs

· Comfortable working independently while also a strong team player

· Demonstrates cultural humility

· Able to keep a sense of humor and “go with the flow”

· Excellent computer skills and ability to utilize a Microsoft Access-based case management system
