The Adult Mental Health Team Lead works under the leadership of the Program Manager of Adult Mental Health and within federal, state and local guidelines performs case management and rehab services with adults, as well as team lead duties. Work involves developing and maintaining long-term contact with clients, client families/LAR, and service providers for medical, social, educational, and related service needs.

This position is responsible for overseeing the care of adults diagnosed with a mental illness and assigned to various Levels of Care (LOC). This position ensures continuity of care for individuals. Team lead duties include training new staff on policies and procedures, workflows, day to day job functions and responsibilities, reviewing and auditing staff notes, scheduling one on one or group training activities, review audit CAPS and follow up with staff, and providing guidance and support to team. This position works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Knowledge, Skills, and Abilities

· Knowledge of community resources, of case management delivery systems, and of program policies and procedures.

· Skill in active listening, de-escalation techniques, empathy, and basic knowledge of social services systems. Skill documenting services provided, conducting assessments, and developing recovery plans. And skill in communicating with a variety of individuals from various backgrounds and experiences.

· Ability to:

o assess clients' needs, coordinate individuals' services, communicate effectively, and to train others.

o work as a member of an interdisciplinary team to ensure referral to appropriate Center program as needed.

o work independently and make decisions but seeks guidance when needed.

o make decisions based on education and or clinical experience, which reflect consistency with Center's Guiding Principles, Policies and Procedures, and Standards of Care.

o work with clients of different cultural backgrounds, sexual orientation, and belief systems.

o work with clients who may display symptoms of mental illness including; poor hygiene, difficulty controlling anger and low frustration tolerance, difficulty or inability to display appropriate social behavior, and who are suicidal or homicidal.

o to engage with individuals served in community locations, including but not limited to residences, community businesses, and local hospitals.

· Understanding and practice of Trauma Informed Care principles.

Essential Duties

Below is a list of essential duties of the position. Other duties may be assigned as needed.

1. Works with Program Manager to develop, schedule, and run team meetings.

2. Handles calls from clients or others related to complaints or problems/concerns with services or treatment, and contacts Program Manager and Rights Department where needed.

3. Provide skills training as appropriate to individuals identified on caseload approved for services as defined in Texas Resilience Recovery (TRR) guidelines.

4. Administers clinical assessments and paperwork to determine appropriateness of level of care assignments required to meet individual's needs.

5. Is familiar with all applicable Texas Administrative Code (TAC), fidelity tools, and Texas Human Health Services (HHSC) contract information and Center policies and procedures.

7. Completes Team lead duties.

8. Meets with Program Manager of Adult Case Management monthly to discuss team productivity and any issue that would require supervisors immediate attention..

9. Works with peers to develop monthly/quarterly team trainings and team building related to topics associated with primary job roles and responsibilities.

Job Type: Full-time

Pay: $21.00 - $23.00 per hour

Expected hours: 40 per week
