Job DutiesHiram W. Davis Medical Center, located in Petersburg, Virginia, is a 94-bed medical facility providing acute medical diagnoses, skilled care, intermediate care, and long-term care to individuals receiving services in DBHDS facilities and within the community that have intellectual and/or developmental disabilities. Admissions to HWDMC are generally either permanent placement or special hospitalization. Patients typically come to HWDMC as a special hospitalization for step-down to stabilize, post-surgery rehabilitation, or long-term care.
The Transition of Care Registered Nurse will oversee the four (4) general medical hospital beds that the Joint Commission regulates by being engaged in direct nursing care and implementing treatment plans for the assigned patients. This Transition of Care RN is to advocate for the patients and staff by collaborating with peers of the nursing leadership team. This nurse will assist in quality assurance activities and assures compliance with agency, departmental, and accrediting agencies' policies, procedures, rules and regulations. The Transition of Care RN is to educate, coach, and lead a productive unit to fulfill the needs of the patients. Also, this nurse will be responsible for admitting and discharging general medical patients internal and externally. Providing primary care nursing by implementing and evaluating nursing care needs daily. Evaluate and supervise the patient care attendants and the overall care for the general medical patients.
Minimum Qualifications
  • Licensed or eligible for licensure as a Registered Nurse in Virginia
  • Graduation from an accredited School of Nursing
Additional Considerations
  • ACLS and/or BLS Certified
  • Experience in nursing at the professional level coupled with experience in medical/surgical, psychiatric, geriatric, rehabilitative nursing, and/or nursing care of the intellectual disability.
