DescriptionYou will play a crucial role in planning, coordinating, and providing hospice care in accordance with the patient’s plan of care. This position requires a Registered Nurse licensed to practice nursing in Arizona, who will work collaboratively with team members and family members to ensure that patient needs are met.

Responsibilities
- Coordinate hospice care with team members and family to meet patient needs.
- Evaluate the effectiveness of hospice services for patients and families on an ongoing basis.
- Perform evaluations for hospice care and informational visits as needed.
- Utilize the nursing process to provide professional nursing care.
- Consult with the attending physician and Hospice Medical Director regarding changes in patients’ conditions.
- Submit complete and accurate clinical notes in a timely manner.
- Participate in IDG and case conferences, and actively engage in QAPI meetings.
- Provide education to patients and family members regarding the dying process and healthcare decisions.
- Conduct comprehensive assessments of patients and their environments.
- Supervise and evaluate the care given by the Hospice Aide.
- Graduate from an accredited School of Nursing.
- Licensed in the State of Arizona as a Registered Nurse.
- One year of RN experience; hospice experience preferred.
- Excellent assessment and problem-solving skills.
- Strong organizational skills and knowledge of case management processes.
- Licensed driver with an insured automobile in good working order.
- Effective communication skills, both verbal and written.
- Ability to administer pharmaceuticals and perform nursing procedures within the scope of practice.
- May be stressful and requires accessing patients’ homes, which may not be wheelchair accessible.
- Physical activities include walking, sitting, standing, and lifting patients as needed.
- Proficient use of computer, telephone, thermometer, B/P cuff, glucometer, and other medical equipment.
- Access to patient medical records is required.
