As a Hospice Registered Nurse/Case Manager, your responsibilities will include:
- Providing and coordinating hospice care in line with individual patient plans
- Collaborating with team members, patients, and families to ensure needs are met
- Evaluating the effectiveness of hospice services and making appropriate adjustments
- Maintaining accurate clinical notes and patient visit schedules
- Participating in interdisciplinary group meetings and case conferences
- Educating patients and families on end-of-life care, healthcare decisions, and dietary counseling
To Qualify For This Role, You Must Have
- Graduated from an accredited School of Nursing
- Valid RN license in Arizona
- At least one year of RN experience; hospice experience preferred
- Excellent assessment and problem-solving skills
- Strong organizational and time management abilities
- Effective communication skills, both verbal and written
