The LPN is responsible for identifying patient/family needs, and for providing supportive care, in accordance with the attending physician's orders and the interdisciplinary group plan of care. The LPNĀ reports to the Clinical Coordinator and Program Director for the provision of evaluation and care in order to maximize the comfort and health of patients and families consistent with Dependable Hospice Care policies and procedures.

We are currently seeking a self-motivated, dynamic, and caring individual to join us as a Licensed Practical Nurse (LPN) in Tucson, Arizona area.

Qualifications:

  • Current licensed as an LPN/LVN in the State of Arizona
  • Some after hours and weekend coverage is a must
  • Demonstrates good verbal and written communication, and organized skills
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
  • Possesses and maintains current BLS certification
  • Demonstrated skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
  • Currently possess or able to obtain an Arizona class 1 fingerprint clearance card
  • Hospice experience is a plus

Responsibilities:

  • Identifying patient/family needs, and for providing supportive care, in accordance with the attending physician's orders and the interdisciplinary group plan of care
  • With the assigned Nurse Case Manager, assume responsibility that includes the assessing, phases of the nursing process
  • Initiate communication with attending physicians, other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family
  • Maintain regular communication with the Nurse Case Manager, Program Director, and Medical Director
  • Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and report to the assigned Registered Nurse Case Manager
