**Job Overview:** We are seeking a detail-oriented MDS Coordinator to join our healthcare team. The MDS Coordinator will be responsible for coordinating and completing the MDS assessments and care plans in accordance with federal and state regulations. **Responsibilities:** - Conduct comprehensive assessments of patients' needs - Develop individualized care plans based on assessment findings - Ensure accuracy and timeliness of Minimum Data Set (MDS) submissions - Collaborate with healthcare providers to optimize patient care - Monitor and update patient records as needed - Stay current on regulations and guidelines related to MDS assessments **Experience:** The ideal candidate should possess the following skills and experience: - Experience in home care, pediatrics, primary care, or medical management - Proficiency in medical coding systems - Familiarity with Epic or other electronic health record systems - Knowledge of outpatient services and medical office operations Join our team as an MDS Coordinator to make a positive impact on patient care while utilizing your expertise in medical coding and care coordination.

Pay: $45.00 - $53.06 per hour
