Job Description:

Devcare Solutions, a leading healthcare provider dedicated to enhancing the well-being of our community, is seeking a compassionate and skilled Social Worker to join our team. The Social Worker will be responsible for assessing clients' needs, developing care plans, and providing support to individuals and families facing social, emotional, and financial challenges.

Responsibilities:

• Conduct comprehensive assessments to identify clients' strengths, needs, and goals.
• Develop and implement individualized service plans in collaboration with clients and their support networks.
• Provide counseling and support to individuals and families to address various social, emotional, and behavioral issues.
• Advocate for clients' rights and access to community resources and services.
• Coordinate with healthcare providers, social service agencies, and other professionals to ensure holistic care.
• Maintain accurate and confidential client records and documentation.
• Monitor client progress and adjust care plans as needed.
• Participate in case conferences, team meetings, and professional development activities.

Qualifications:

• Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution.
• State licensure or certification as a Social Worker (LCSW, LMSW, etc.).
• Proven experience in social work or related fields, preferably in a healthcare setting.
• Knowledge of social work principles, practices, and ethics.
• Strong interpersonal and communication skills.
• Ability to work effectively both independently and as part of a multidisciplinary team.
• Proficiency in electronic health records (EHR) and case management systems.

Job Type: Contract

Pay: Up to $33.00 per hour

Expected hours: 40 per week
