Qualified Program Specialist (Behavioral Support Professional)

As a Qualified Program Specialist (QPS) at Development Homes, Inc., you empower individuals with disabilities to achieve their unique life goals and outcomes, ensuring they lead fulfilling lives within their homes and communities. Your primary responsibility is to support individuals in identifying, pursuing, and attaining their intended outcomes while ensuring compliance with Personal Outcome Measures (POM) and Title 19 regulations. Acting as a liaison between the individual, their family, guardians, and other support networks, you will facilitate the development and execution of comprehensive Life Plans tailored to each person's unique needs and desires.

Essential Responsibilities

1. Person-Centered Support

  • Support individuals in achieving their unique life goals and desires.
  • Demonstrate and apply knowledge of Personal Outcome Measures and Title 19 regulations.
  • Educate and guide other staff in understanding and implementing Personal Outcome Measures.

2. Liaison and Coordination

  • Act as a liaison between individuals, their families, guardians, and other support networks.
  • Provide leadership and coordination for the annual Life Plan and other necessary meetings.
  • Facilitate team meetings with planned agendas, ensuring all meetings are documented and filed appropriately.

3. Life Plan Development

  • Develop Life Plans based on established personal outcomes and objectives.
  • Conduct annual self-assessments and update plans throughout the year, incorporating feedback from individuals.
  • Ensure Life Plans are developed, distributed, and filed within 30 days of the meeting.

4. Monitoring and Assessment

  • Monitor the implementation of Life Plans to ensure supports are in place and effective.
  • Conduct quarterly reviews to measure progress and document outcomes, distributing findings to team members and entering them into a computer database for tracking.

5. Health and Safety

  • Ensure the basic health and safety needs of individuals are met, adhering to all work safety rules.
  • Provide personal care support as needed to ensure a comfortable, safe, and sanitary living environment.

6. Communication and Documentation

  • Communicate relevant information regarding individuals and operations both verbally and in writing.
  • Maintain positive interactions with other agencies and providers.
  • Adhere to HIPAA privacy and security regulations, monitoring compliance.

7. Training and Development

  • Ensure all staff training is conducted and documented as required.
  • Actively participate in the development and implementation of Competency-Based Training for caseload work sites.
  • Maintain current training certifications and attend annual in-services as required for self-improvement.

8. Personal Care and Community Integration

  • Support individuals in participating in community activities and enhancing relationships.
  • Assist and support individuals in their day-to-day routines within their home and community.

9. Responsiveness and Flexibility

  • Be flexible and responsive to the needs of your caseload, and cover shifts outside of business hours if necessary.
  • Balance team and individual responsibilities while ensuring quality and timeliness of tasks.
  • Perform other duties as assigned by the Residential Services Director and/or Adult Services Coordinator.
  • Participate in rotating weekend/holiday and storm team on-call responsibilities.

10. Financial Management

  • Support individuals and Residential Managers with developing personal budgets and ensuring benefits are maintained and current.
  • Assist in the annual development of group home budgets and ISLA contract updates to address the needs of individuals.

Basic Qualifications:

  • High school diploma or GED.
  • Must be 18 years of age or older.

Educational Qualifications:

  • Must possess a B.A. or B.S. degree in Human Services field; such as Social Work, Psychology, Health Care, Counseling, Sociology, and Family Services.
  • Minimum of two years of experience in a human services-related field is required.
  • Developmental Disabilities Certification from Minot State University is preferred.

  • Team Collaboration: Work collaboratively with team members while maintaining individual accountability

Additional Requirements

  • Availability to work Monday to Friday from 8:00 am to 5:00 pm. Must be flexible being on-call and cover shifts to assist working with individuals supported outside business hours including evenings, overnights, early mornings, holidays, and storm days if necessary.
  • Position requires successful completion of criminal background, child abuse and neglect check, credit check, and a pre-employment drug test to be eligible for employment.
  • Current and valid driver’s license, proof of auto insurance, and insurable driving record following company guidelines.

Compensation and Benefits:

Qualified Program Specialist (QPS): Starting annual wage for this position is $47, 179 (plus applicable differentials).
