Developmental Pathways (DP) has an opportunity for a Service Coordinator (SC) in our Early Intervention Department . This position will support services or therapeutic intervention for children living in a variety of settings. Communicates with a broad team of providers which may include professionals, direct care employees, families, and natural supports chosen by the individual/family being served. The Service Coordinator provides leadership and mentoring to Coordinator positions and problem-solves difficult situations with the management team, families, and providers
This is a 40-hour week, hybrid position, working primarily remotely but coming into our headquarters office in Aurora, CO, and/or other offsite locations as needed.

Requirements
Bachelor’s degree, OR
Five (5) years’ experience in the field of developmental disabilities, OR
A combination of education and experience appropriate to the requirements of the position
Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years
Preferred
Experience with Child Find and/or Part C laws and regulations
Working knowledge of the developmental disability system, rules, and regulations
Additional Responsibilities
Coordinates potential services and/or providers to meet individual needs as determined in the individual plan (IFSP)
Develops and reviews IFSP/FSP with family and multidisciplinary team
Schedules and facilitates meetings with individuals and families in services
Counsels individuals and families to determine priorities for services
Organizes new service request(s) to appropriate entity for eligibility determination and provides support throughout that process
Monitors health and safety issues, advises and counsels as necessary; facilitates referral to other agencies
Ensures Division of Intellectual and Developmental Disabilities/DIDD rules and regulations, Part C law, and agency policies and procedures are adhered to in the provision of services
Attends and presents at resource fairs, CICC meetings, and public awareness events to communicate information about programs and services
Document case notes, TCM notes, and data entry in UNICORN in a timely manner as outlined by department expectations
Serves as a liaison to effectively communicate and work with a large cross-section of stakeholders including Pathways departments and outside entities to interpret documents, request data updates, and convey program/policy changes
Actively develops collaborative relationships with providers and Child Find staff and other community partners; participates in the state and local early childhood community task forces, work groups, advisory boards, and the Early Childhood Council
Problem solves, explores, communicates, and implements creative avenues to increase revenue for direct service and service coordination and ensures providers are paid appropriately and timely in accordance with the individual’s IFSP/FSP
Integrates knowledge of rules, regulations, and requirements related to serving diverse populations and the coordination of unusually challenging and complex client issues
