We are looking for a case manager that will Work with patients to develop individualized case management plans, to assist with both immediate and long term needs including housing, county benefits, employment etc; Ensure that all patient case management plans address identified needs and are up-to date; Continually monitor and evaluate each patient's progress in achieving goals and develop corrective action revisions to the plan as needed; Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County guidelines; Assist patient's in obtaining all available resources to achieve case management goals; Actively participate in patient- and program-related meetings and training; Attend program-related community, coalition and committee meetings as assigned; Maintain patient related data systems, including case notes and Prepare case management related reports including but not limited to: outcomes, successes, etc; Adheres to strict boundaries and professional ethics in the care of others; Conducts crisis intervention as needed.*

Job Type: Full-time

Pay: $22.00 - $30.00 per hour
