This posting will close at 500PM on the close date.This Position Is Grant Funded With BenefitsUnder the supervision of the Court Compliance Division Manager or the designated program administrator this position provides community support services and care navigation services to people involved in or at risk of becoming involved in the criminal justice system.
Resource Navigation
  • Works with clients to identify health, social, economic, and educational needs, by conducting client assessments and ongoing interactions with the clients.
  • Educate clients about treatment programs, public assistance programs, and other resources to improve access to care and support recovery.
  • Links clients to resources in the community to meet needs through referrals and warm hand-offs with community organizations. May include coordinating transportation to appointments or attending appointments with clients.
  • Understand and promote person-first, recovery-orientated approaches, and services.
  • Document clients’ progress and maintain proper records to track adherence to court orders or engagement with care plans, as well as maintain confidentiality.
  • Assist clients with access to healthcare and social services in an appropriate setting, and advocate for clients if needed.
Client and Community Engagement
  • Schedule home or community visits to engage with clients and assess appropriate living conditions and support client engagement.
  • Conduct regular face-to-face meetings with clients, attend community events, participate in professional training, and build relationships with community providers.
  • Network with community-based organizations to identify available resources and develop partnerships to benefit clients.
  • Identify and address needs and barriers to support access to services and recovery.
  • Maintain client-centered focus and non-stigmatizing communication, focusing on harm reduction when needed.
  • Create plans to reengage clients that are most essential to serve, but may be unable to access services, including motivational interviewing and brief interventions.
ADDITIONAL DUTIES. Performs Other Duties As Assigned.Education. Associate degree in Psychology, Social Work, Family and Consumer Science, Counseling, Liberal Arts, Humanities, Sociology, or related field. Certified as a Peer Support Specialist, Community Health Worker or Family Peer Support Specialist is preferred.
Experience. One (1) year of case management or peer support experience. Bilingual (English/Spanish) is required.
Education/Experience substitution. In accordance with County policy.
Licenses/Certifications. Valid Driver’s License required.
Other (e.g., post-offer medical exam, polygraph, background check, driver’s license record, etc.). Must pass background and driver’s license check. Must maintain an acceptable driving record in accordance with County Policy.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED BY THE POSITION Must have knowledge of peer support and recovery-oriented support, healthcare and social services systems, motivational interviewing technics, and general knowledge of community resources.
Must have skill to operate a computer using software including Microsoft Office Suite; maintain accurate records; communicate effectively in verbal and written form with others.
Must have ability to work independently; observe clients, recognize behavior changes, and take effective action; prioritize and multi-task; solve problems; interact with a wide range of people; develop collaborative partnerships; and maintain confidentiality.
