Transition Specialist – Juvenile Detention Education Program - Douglas ESD Douglas ESD is seeking qualified applicants for the position of Transition Specialist for the Juvenile Detention Education Program. This position works 190 days per year, 8 hours per day. The overall goal is to assist students exiting the Juvenile Department Education Program with transitioning to their next educational setting and/or securing appropriate, meaningful employment and/or post-secondary education or other appropriate educational placements to ensure success and independence. This specialist will work with other educational settings, teachers, students, parents, businesses, and other community members to develop an integrated transition system for the purpose of ensuring a strong focus on academic success. The position is designed to help educational programs and teachers understand and embrace the philosophy that integrated work-based and career education learning strategies is an essential component of public education. In coordination with local and state education improvement efforts, the specialist will develop multiple strategies to deliver and support change within schools and communities while supporting youth in their transition back to their school and community. DUTIES:This is not an exhaustive or all-inclusive list of responsibilities, skills, duties, or functions associated with the job. Full job description is attached in Applicant Tracking. Serve as a liaison between school administrators, counselors, and teachers to ensure the successful transition of students in the program to their schools.Responsible for presenting IEP’s, behavior plans or other relevant documents to the appropriate staff of the receiving schools.Ability to coach staff from the receiving schools on how to best work with the student making a transition to their school.Ability to coach students on the rules of the receiving school and help them adjust to the school’s culture and communicate how to use coping skills and positive behavior interventions per their individual plan.After a successful transition takes place will be responsible to follow up with the school to ensure the student is making adequate and successful progress toward goals.Develops contacts with area businesses and government agencies and recruits business and government participation.Provides students with career and college information, serves as a liaison between schools and agencies and guides students in successfully reaching determined goals.Explain format and content guidelines for resumes, cover letter, and other work portfolio activities to students.Gathering and providing school documentation of any student’s disability, key information, and other relevant information that can be found in the student’s file and/or IEP to assist in determining the student’s eligibility for other agency services.Assist students with necessary other agency applications to help acquire transportation in order to keep necessary appointments to assess their eligibility.Meet with necessary agencies, as requested, to provide updated information on student progress and status of completion of secondary school, employment, or post-secondary activities, as well as providing updates on any change in student status that would impact successful completion of the IEP or other requirements.Ability to work a flexible schedule and travel to and from school locations in the Douglas ESD service area and may be required to travel on overnight assignments.Other duties as assigned EDUCATION AND/OR EXPERIENCE: Experience working with secondary transition-age students and an understanding or experience working with outside agencies or providers.
