Job Description: PURPOSE OF JOB : The Patient Safety and Quality Improvement Specialist is responsible for facilitation and oversight of safety and quality performance improvement initiatives throughout Doylestown Hospital. This individual will be tasked to improve quality of patient care through process reengineering, safety strategy optimization, utilization of quality improvement tools and comprehensive data management and analysis. ESSENTIAL FUNCTIONS: -Strategically develop, evaluate, and execute the organizational quality, patient safety and performance improvement program in partnership with the Director of Quality Initiatives. -Respond to events by conducting thorough event investigations, completion of Root Cause or Apparent Cause Analyses as appropriate, partnering with internal experts to identify and implement system improvements, as well as maintenance of ongoing tracking for trends. -Collaborates with multidisciplinary teams through a disciplined approach to achieve desired improvement results, including identification of process and outcomes measures, establishment of data collection tools and strategies, collection of baseline data, Pareto and other analyses to establish areas of improvement focus. -Provides leadership for QAPI projects through partnership and coaching of departmental teams in all phases of project management for the achievement of their goals. -Performs chart reviews and clinical evaluations for determination of quality excellence.
-Updates organizational leadership and stakeholders regarding ongoing improvement goals, progress, identification of inherent safety risks and hazards as well as mitigation strategies.
Job Qualifications: QUALIFICATIONS : Education:
-BSN is required, Master's degree preferred
-Certification in patient safety or CPHQ preferred
-Licensure: RN required Experience: 3- 5 years experience in performance improvement project management and patient safety strongly preferred Other Skills:
-Initiative, judgment, creativity and leadership abilities
-Experience in use of Model for Improvement, Lean Six Sigma, PDSA preferred
-Competence in application of tools such as FMEA, RCA, PRA, process mapping preferred
-Data management and analytics, expertise in development of process and outcome metrics
-Facilitation of multi-disciplinary teams, strong interpersonal skills
