General Description of the Job Class
The rheumatology clinical pharmacist provides clinical pharmacy services to patients served by Duke Rheumatology. The clinical pharmacist shall provide services for rheumatology disease states. The pharmacist may support the development of therapy plans, facilitate medication access, provide patient education, support laboratory monitoring requirements, serve as a resource for healthcare providers and staff, and facilitate utilization of Duke retail and specialty pharmacies, as appropriate. A clinical practice as a clinical pharmacist practitioner may be maintained virtually and/or at an outpatient clinic. The position may also include precepting pharmacy students and residents.
Duties and Responsibilities of this Level
Clinical Pharmacist Responsibilities
Clinical Practice:
  • Participates in clinic and care management activities to provide clinical pharmacy services and oversight for safe use of medication. This may include prescribing and monitoring of medications within the scope of practice designed collaboratively with physician colleagues. This may include, but is not limited to:
    • Completes/Reviews medication reconciliation.
    • Provides medication counseling.
    • Prescribes and adjusts medications as permitted through collaborative practice agreement.
    • Screens for drug interactions, therapeutic duplications, omissions, etc.
    • Monitors patients for therapeutic and/or adverse response to medication
    • Coordinates patient care and ensures appropriate follow-up, including laboratory monitoring, with other outpatient or inpatient teams.
    • Ensures continuity of care and access with all medications.
    • Facilitates prescription fulfillment through Duke specialty and retail pharmacies, as appropriate. Coordinates with Duke specialty and retail pharmacies, including patient assistance and prior authorization teams, to provide seamless medication access to all patients. Coordinates with external retail pharmacies as needed. Coordination may include supporting prior authorization process as needed, among other activities.
    • Supports Duke Specialty Pharmacy patients by serving as the Duke Specialty Pharmacy on-call pharmacist on a rotating schedule with other pharmacists in the Duke Specialty Pharmacy and specialty clinics.
    • Collaborates with infusion pharmacy staff to provide continuity of care on medication infusion related issues as necessary.
    • Documents the care provided in patients' records.
    • Participates in interdisciplinary reviews of patients' progress, when applicable.
    • Serves as resource to providers, care managers, and other healthcare professionals to provide medication reviews and to assist in accurate completion of medication reconciliation activities.
    • Serves as resource to assist in addressing medication access and medication adherence challenges. Coordinates and collaborates with Duke pharmacy technicians to overcome these barriers, as appropriate.
    • Refers patients to patient assistance programs, or other resources to navigate medication access issues, as needed.
    • Proactively assesses and encourages appropriate medication adherence.
    • Serves as a resource to providers and staff for timely general drug information and payer pharmacy policy.
    • Provides educational in-services on medication-related topics to patients, providers, staff, care managers, and other healthcare professionals.
    • May provide discharge counseling and education for select high-risk patients, either face-to-face or telephonically.
    • May provide both direct patient care and virtual services to patients as determined based on oversight from rheumatology and pharmacy leadership.
    • Interprets data related to medication safety and effectiveness. Provides support to improve safety and reduce risk to patients. Reports events and/or concerns via the Duke Health Safety Reporting System (SRS).
    • Provides oversight of cost-effective use of medications and treatments. This includes risk reduction and correct utilization of resources to optimize care.
    • Participates in developing guidelines, algorithms, and care pathways to improve the care and safety of patients.
    • Participates in efforts to maintain TJC accreditation standards within the realm of medication use at clinical practice sites.
    • Serves as a resource for staff with the Duke specialty and retail pharmacies for rheumatology related medication issues.
    • Represents specific concerns for Information Technology applications.
    • Develops and participates in pharmacy or medication-related related research projects and quality improvement initiatives.
    • Educates medical, nursing, and pharmacy staff about the management, prescribing and monitoring of drug therapies.
    • Provides educational lectures and/or participates in research (Duke, State/Local, National).
Administrative Practice:
  • Participates in Duke Health committees and initiatives related to rheumatology, specialty pharmacy, transitions of care, and/or pharmacy practice.

  • Collaborates with peers to achieve department and organizational goals.

  • Coordinates with others to maintain up-to-date knowledge of pharmacy benefit design and access in support of pharmacy practice.

  • Maintains familiarity with pharmacy programs across Duke Health and functions as an integral member of the pharmacy team.

  • Models behaviors that exemplify Duke Health values, including teamwork, integrity, diversity, excellence and safety.

  • Maintains outstanding skills and consistently demonstrates the value of pharmacy services.

Minimum Qualifications
Education
  • Doctor of Pharmacy Degree OR BS Pharmacy Degree
Experience
  • With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.
  • With Doctor of Pharmacy degree, no experience is required.
  • PGY1 or Pharmacy Practice Residency preferred; with 1 years of clinical practice experience or experience in managing rheumatology patient populations preferred.
  • PGY2 Ambulatory Care Residency and BCPS or BCACP preferred.
  • 2 years of clinical practice experience or experience in managing rheumatology patient populations preferred.
Degrees, Licensures, Certifications
  • Licensed Pharmacist or eligible to practice pharmacy in North Carolina
