Job Description And ResponsibilitiesThe primary purpose of Universal Care Partner is to work collaboratively as part of a household team to create an environment that encourages resident involvement and personal growth. Under the supervision of a registered nurse, the primary responsibilities of this position include delivery of exceptional nursing care in a household environment, assistance with the delivery of an overall dining experience and the collaborative evaluation, assessment and implementation of programming that will mirror normal life activities of residents (to include the upkeep and management of personal living space) mental and emotionally engage their interest and assist them in recapturing the value of normal community life. Performs all job responsibilities according to Duncaster's Policies & Procedures. Duties And ResponsibilitiesThe following description of job responsibilities and standards are intended to reflect the major responsibilities and duties of the job but is not intended to describe minor duties or other responsibilities as may be assigned from time-to-time.
Provide exceptional person-centered care in a household environment.
Create and maintain an atmosphere of warmth, personal interest and positive emphasis" as well as "a calm environment throughout the household.
Review the resident's plan of care with the charge nurse with any new assignment, changes or when in need of additional explanation or teaching.
Provide personal hygiene (bath, shower, shampoo, assistance with elimination needs, dressing, or other personal care to include shaving and fingernail filing) as directed by the plan of care.
Perform safe transfers and assists with ambulation safety using assistive devices such as cane, walker, wheelchair, mechanical chairs, etc.
Takes vital signs as per plan of care; reports any change of condition to charge nurse.
Competency is using electronic medical record system.
Complete all required documentation both in the resident's apartment/room as well as any other form such as safety check tracking or as needed for billing.
Assessment and implementation of programming that will mirror normal life activities of residents (to include the upkeep and management of personal living space) mentally and emotionally engage their interest and assist them in recapturing the value of normal community life.
Assist residents with meals, including food preparation as needed, monitor dining room, assist in serving, assists with menu selection and submit menus to dining services.
Complete basic household chores to include making bed, laundry, trash removal, washing dishes, checking refrigerator for spoiled food, etc.
Prepare residents for appointments and assist with transportation within the community; accompany to outside appointments as needed.
Respond to "call for aid" as directed by the charge nurse or other person in charge.
Utilize standard precautions at all times and report a potential exposure or hazardous condition to charge nurse.
Complete all required education programs and complete at least 8 hours of dementia education annually.
Demonstrate skills specific to providing care to residents affected by dementia.
Education And Experience
Current State of Connecticut certification as a Home Health Aide or a Certified Nursing Assistant.
Must be eligible for Certified Nursing Assistant registration in the State of Connecticut.
Must have the ability to read, write and speak English.
Must have a high school education or equivalent.
Must be able to safely perform the essential job functions with or without reasonable accommodations.
Must have certification in Serve Safe handler.
