Location: Brooklyn, NY | Schedule: Full-time, Monday-Friday 9:00AM-5:00PM | Benefits: Medical/Dental/401K, Ongoing Paid Time Off (PTO)
Primary Purpose Of The RoleAs a Forensic Case Manager at EAC Network, you will be a key player in the Manhattan Health Court Evaluation Services program. Your primary responsibility will be to provide services such as identification, screening, treatment planning, case management, and court reporting for eligible participants living with mental illness or co-occurring disorders.
Key Responsibilities
- Serve as a vital link between the courts, correctional facilities, and program participants
- Conduct case-finding and screening to develop tailored service plans
- Implement comprehensive screening procedures and facilitate client referrals to suitable treatment facilities
- Submit monitoring reports to the MMHC Resource Coordinator in a timely manner
- Maintain complete and up-to-date case documentation for review
- Assist clients in accessing public and private benefits
- Monitor client participation in treatment programs and conduct regular case management appointments
- Prepare and present status reports regarding client progress to the criminal justice system
- Ensure regular contact with program participants and treatment providers
- Attend training sessions and continuing education programs as directed
- Provide guidance to staff members on treatment planning and needs
- Assist with administrative tasks related to program management
- Bachelor's degree required; Master's degree preferred in criminal justice, social work, counseling, forensic psychology, psychology, sociology, or criminology
- Minimum one year of experience working with criminal justice, substance abusing, or mentally ill populations
- Must be fluent in English and Spanish
- Strong communication skills and ability to collaborate effectively with treatment and criminal justice systems
- Proficiency in computer applications and Microsoft Office software
