East Neck Nursing & Rehabilitation Center is looking for a Full Time RN Supervisor to join the Nursing Team!

Position Summary:

The RN Nursing Supervisor is responsible for planning coordination, and direction of all nursing staff on all shifts on her assigned units, as well as, ongoing general evaluation of staff performance and activities, in order to achieve optional resident/patient care.

In this position, the RN Nursing Supervisor interacts with all levels of nursing personnel, other department personnel, and medical providers to assure quality care is provided to each resident/patient.

Qualifications:

  • Must have 2-3 years experience in Long Term Care & Nursing Home as an RN .

EDUCATION:

  • New York State Licensed Professional Nurse
  • PRI Certified
  • Proficient in MDS completion and Care Planning

PHYSICAL DEMANDS:

· Must be able to move intermittently throughout the day.

· Must be able to speak and write the English language in an understandable manner.

· Must possess sight/hearing senses or use prosthesis that will enable these senses to function adequately so that the requirements of this position can be fully met.

· Must function independently, have personal integrity, have flexibility, and the ability to work effectively with other personnel.

· Must e able to push, pull, and move things.

· Must be necessary to assist in the evacuation of residents during emergency situations.

Duties and Responsibilities: Every effort has been made to keep your job description as complete as possible. · Participates in formulating, implementing and interpreting nursing policies and procedures.

· Promotes and evaluates staff adherence to all Nursing and Facility policies and procedures.

· Responsible for daily monitoring of electronic medical records dashboard (including “alerts”,, incomplete UDAs, Immunization entries, Medication/Treatment alerts, etc.) and addressing any issues identified.

· Conducts rounds on all residents on her/his assigned units. Will review the chart documentation on all the residents in the 24-hour report.

· Ensures new admissions, discharges and transfers are entered in the Electronic Medical Record (Quick ADT) as soon as it occurred.

· Reviews the 24-hour census to assure accuracy for staffing and billing purposes.

· Conducts documented unit in-services on documentation when medical record reviews demonstrate confusing, conflicting or incomplete documentation.

· Provides assistance and direction to nursing personnel in assessing residents/patients and units needs.

· Reviews C.N.A.s Accountability and Assignment records for accuracy of assignment in accordance with resident/patient needs, and completeness of accountability documentation.

· Reviews documentation on residents receiving restorative physical and/or occupational therapy programs in order to ensure rehabilitative criteria is met.

· Completes MDS as required for all new admissions, quarterly, annually and at significant change assigned.

· Works with unit licensed nurses to ensure documentation reflects residents/patients need in all aspects related to MDS and PRI completion on an ongoing basis.

· On an going basis, conducts medical record documentation inability to “capture” the resident’s /patient’ needs, and In-service as needed.

· Reviews each resident/patient accident/incident report; initiates investigations of injury of unknown origin; and conducts abuse investigations when applicable.

· Conducts and completes a staff competency assessment when resident/patient sustains an injury during care routine.

· Submits accident/incidents report and completed applicable investigation reports to the Associate DNS, within 48 hours of occurrence.

· With the unit licensed nurse, ensures care plans reflect the current updated status of resident/patients, to include infections, accident/incident, etc.

· Monitors the implementation and effectiveness of the Nursing Rehab program for each resident, at least monthly, with corresponding documentation.

· Ensures the comprehensive care plan reflects the Nursing Rehab goals and interventions.

· With the unit licensed nurse, assesses each resident’s skin integrity, and develops necessary plans of care.

· Completes and submits a pressure ulcer protocols in progress.

· Makes necessary recommendations to the medical provider, related to changes in condition.

· Initiates ad hoc care plan, meetings with Dietary, Medical and Rehab disciplines to ensure all are involved in pressure ulcer treatment, healing, and/or prevention.

· Ensures care plan reflects current status and updates to the pressure ulcer plan of care.

· Conducts continuous quality improvement audits, as assigned, and/or by self-identification of specific problems.

· Collaborates with the Staffing Coordinator in planning for staffing needs of nursing units.

· Monitors staff attendance and punctuality in reporting to the nursing unit.

· Plans adequate nursing coverage in non-unit areas during resident/patient activities, e.g., Smoke Room.

· Prepares and conducts performance evaluations of nursing personnel under his/her responsibility.

· Collaborates with Nursing Administration regarding staff discipline and carries out directives as indicated.

· Serves as resources person to other disciplines where nursing expertise is indicated.

· Participates in Facility committees as assigned.

· Endorses, interprets and supports Facility and Nursing Philosophy, goals and objectives.

· Maintains a well-groomed, clean appearance; adheres to the Facility dress code; wears the employee I.D. in a visible manner with picture and large print facing outward.

· Assists other team members in the performance of their assignments.

· Assumes other team members in the performance of their assignments.

· Assumes additional responsibilities as delegated by the Associate/Assistant Director of Nursing Services.

· Respects the patient’s rights to confidentiality at all times.

· Displays sensitivity to the needs of patients, visitors, and co-workers.

· Greets and interacts with others with consideration, courtesy, and respect.

· Communicates effectively with patients, visitors, and co-workers, in a clear, timely and professional manner.

· Performs work duties willingly and with initiative.

· Remains calm, and continues to work effectively in stressful situations.

· Displays friendly helpful behavior to patients, visitors, and co-workers.

· Demonstrates judgment and tact when dealing with patients, visitors, and co-workers.

· Uses professional judgment and flexibility in presenting alternatives or innovative solutions to problems encountered.

· Cooperates with other departments or work groups/peers.

· Demonstrates appearance and hygiene appropriate to the delivery of quality services. Maintains professional appearance, and adhering to the state uniform policy including identification badge.

· Complies with fire, disaster, infection control, safety hazard, and emergency procedures.

· Reports to work on time and is ready to work at start of shift; adheres to department schedules.

· Give appropriate notice of absenteeism and does not abuse time off benefit.

· Other duties as assigned.

· Maintains an adequate liaison with families, residents and patients.

· Demonstrates awareness, respect and concern for internal customers (coworkers) and external customers (i.e. patients, residents, families, physicians, referral sources)

Residents' Rights:

· Maintains CONFIDENTIALITY of all resident care information to assure resident rights are protected.

· Assures that the resident's rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are followed.

Attendance:

· Observes rules, policies and regulations with regard to attendance, punctuality, appearance and professional attitude as established by the facility.

Miscellaneous:

· Conforms to the HIPAA Compliance Program and applicable facility policies for patient privacy.

· Conforms to the Facility’s Corporate Compliance Program to include Code of Conduct and Conflict of Interest.

· Conforms to the facility’s policies and procedures.

Physical:

· Able to stand times (2) plus hours without interruptions or breaks.

· Must be able to sit, stand, bend, lift and move intermittently throughout the day.

· Must be able to assist with the evacuation of residents during emergency situations.

· Must pass Medical and Physical examination based on job qualifications, requirements and duties/responsibilities.
