Job SummaryThe FACT Team Lead functions as an administrative lead of the interdisciplinary team with duties including referral management, liaising with referral agents, chairing daily FACT meetings, and coordinating the daily schedule. They coordinate Psychiatry clinics and/or telehealth sessions, consult and collaborate with internal and external services providers as well as coordinate and monitor treatment plans and service delivery. This position demands a high level of independence with work assignments of a complex nature and the use of specialized skills in the treatment of mental illness and addictions within an interdisciplinary team. The incumbent will also provide clinical consultation, leadership, and orientation to other team members. They will assist with program planning, development and evaluation along with adherence, understanding and implementation of the FACT model.
While the primary function of this position is to support operations of the FACT Team, the Psychiatric Nurse IIA/ Team Lead will also have an individual caseload (reduced) along with being actively involved in all high intensity FACT clients. This position works with considerable independence as well as facilitating a collaborative and interdisciplinary work environment. Reporting to the manager of the FACT team, the Psychiatric Nurse IIA, Team Lead is responsible for the development, planning, organization, supervision and evaluation of the care provided by the FACT Team and other duties as assigned. The successful candidate uses extensive knowledge and skills gained through experiences in Psychiatric nursing, Recovery and Psychosocial Rehabilitation principles, to assist clients in meeting their goals.
Job DemandsRequirementsBy applying for this job, you acknowledge an understanding that regular requirements and demands of this job include (but are not limited to):
Regularly moves or lifts between 5 - 10 lbs, occasionally moves or lifts up to 25 lbs and occasionally is required to push patients in wheelchairs over 50 lbs.
Regularly walks , stands or sits to provide care or to observe patients.
Regularly uses fine finger/precision work requiring steadiness and accurate movement/control.
Visual concentration is required to observe a patient's behaviour and body language.
Occasionally required to drive to patients homes for visits or to accompany them to community agencies or resources.
Job QualificationsGraduation from an approved/recognized post-secondary nursing education program with a Baccalaureate degree in Nursing OR a diploma in Nursing supplemented by a Post-Basic course or CNA Certification in Psychiatric Nursing is required.
Practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL).
Active certification in BLS.
Minimum of five (5) years' experience within the last seven (7) years in Adult Mental Health nursing in Psychiatric Rehabilitation, Acute Care or Ambulatory Care/Community Mental Health is required.
Experience and knowledge of adolescent and youth Mental Health.
Demonstrated competencies in organizational, leadership, interpersonal, team building, communication and decision making skills.
A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private vehicle are conditions of employment.
A satisfactory record of work performance and attendance is required
