NEW HIRE SIGN ON BONUS ELIGIBLE POSITION - $8K! POSITION SUMMARY: Provides direct and indirect patient care to Cardiology clinic patients and their families utilizing the nursing processes of comprehensive patient assessment, development and implementation of nursing care plans and patient education, and on-going evaluation. Delivers holistic and individualized care to all patients in practice. Develops, implements, coordinates an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications. Adheres to polices, industry standards, best practices, and applicable laws/regulations and codes to promote a quality, highly reliable patient experience. Engages in continuous growth and development in professional nursing practice. EDUCATION/CERTIFICATION: Must be a graduate of an accredited nursing school and be a licensed State of Connecticut Registered Nurse. Proficient in cardiac monitoring and be certified in Basic Life Support and Advanced Cardiac Life Support. EXPERIENCE: Experience with computer scheduling systems preferred, i.e. Meditech and Allscripts/Veradigm. 2 years experience working in a medical facility. COMPETENCIES: Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people. Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record. Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions. Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines. Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations. Demonstrated knowledge and competence with regard to nursing theories, concepts and practices, medical terminology, anatomy, and physiology. Sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, care, push and /or pull light to moderate amounts of weight; to operate office equipment requiring repetitive hand movement and fine coordination. Ensure the privacy of each patient#s protected health information (PHI). Build collaborative relationships with peers and other healthcare providers to achieve departmental and corporate objectives. Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options. Skilled in identifying patient-related problems and recommending solutions. Skilled in establishing and maintaining effective working relationships with patients, families and the community. Ability to maintain quality control standards. Ability to interpret, adapt and apply guidelines and procedures. Knowledge of common safety hazards and precautions to establish a safe work environment. Provides direct care to patients and makes necessary nursing judgments. Determines the urgency and disposition for telephone triage requests and other patient calls requiring clinical expertise and decision making. Collects pertinent clinical data and applies nursing knowledge and evidence-based assessments to coordinate the delivery of care during telephone encounters, electronic communications and/or clinical visits. Collaborates with the provider and communicates with patients regarding symptom management and the coordination of additional services as needed. Participates in the coordination of care # on-going management of patients by developing shared care plans, performing medication reconciliation, completion of prior authorizations, processing prescription renewals, performing symptom assessments, coordinating referrals and assisting prescribers in tracking and meeting timeframes for annual labs. Participates in in-office procedures, as related to clinic specialty, under the direction and supervision of a Physician or APP and documents results in EHR. Accurately completes necessary documentation of all aspects of the nursing process on an ongoing basis, in a timely manner, as per departmental policy and regularly acts as a resource to others regarding documentation issues. Oversees office Coumadin clinic. Educates patient / family regarding diagnostic procedures, medications, nutrition, and maintenance of health and wellness. Anticipates situations and takes action to prevent or minimize a negative outcome while demonstrating the ability to organize and delegate appropraiately in an emergenc in a calm efficient manner. Maintains/reviews patient records, charts and other pertinent information. Appropriately delegates duties and tasks to team members, developing opportunities to promote growth of subordinates and/or peers. May be required to perform other duties as requested by Management.
