The Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family.
The Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Care Manager helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements and patient satisfaction).
The Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well-organized and safe plan of care is established for every patient.
Minimum Requirements
Education: Associate Degree in Nursing, Bachelors of Science in Nursing, or Bachelors of Science in Social Work required
Experience: 0-3 years of Medical Case Management
Registered Nurse (RN) license is preferred

Department: Care Management (outpatient)
This position is PRN (on an as needed basis) and will cover care management needs in 4 counties (Pitt, Edgecombe, Lenoir and Beaufort) at our outpatient clinics.
Shift Hours: 8:00am- 5:00pm during the week with no call, no nights, and no weekends.
