JOB SUMMARY: The Health Information Management (HIM) Clerk works daily in partnership with the Health Information Management Manager. This position handles organizing, keeping, and securing patients’ health records within Edgewater Health’s facilities. The HIM Clerk also ensures that medical records are correct, complete, and accessible to authorized personnel when needed.
DUTIES AND RESPONSIBILITIES:
- Record Management: Organize, file, and retrieve medical records in physical or electronic formats.
- Data Entry: Input patient data into electronic health record (EHR) systems accurately.
- Compliance: Ensure that health records follow legal, regulatory, and institutional standards, such as HIPPA.
- Patient Information Requests: Handle requests for medical records from patients, healthcare providers, or insurance companies while keeping confidentiality and assuring patient’s rights are always protected.
- Auditing: Conduct regular audits of health records to ensure accuracy and completeness.
- Collaboration: Work closely with healthcare providers, billing departments, and other administrative staff.
- Screening and Indexing: Digitize paper records and ensure they are appropriately categorized in the EHR system.
- Customer Service: Respond to inquiries and aid with the release of information.
- Attending and taking part in workshops, seminars, trainings, as assigned.
- Open episodes for admissions/re-admit, in the DARMHA systems.
- Imports assessment data into DARMHA.
- Chart location data into NextGen and DARMHA.
- Enter discharge data into DARMHA systems.
- Abstract information from records as authorized/needed for insurance companies, Medicare, etc.
- Reviews the completeness of charts for completion and accuracy.
Qualifications

- Attention to Detail: Accuracy in handling sensitive data is critical.
- Organizational Skills: Ability to manage large volumes of records effectively.
- Technical Proficiency: Familiarity with HER systems and office software.
- Communication: Strong people skills for collaborating with patients and staff.
- Confidentiality: Understanding of privacy regulations like HIPPA.
- Problem Solving: Ability to resolve discrepancies in health records.

MINIMUM QUALIFICATIONS:
- Education – High school diploma or equivalent (required).
- Post-secondary certification in Health Information Management or related fields (preferred).
- Experience -- Minimum of two years’ experience working and/or volunteering within a healthcare facility and a good working knowledge of medical records management.
TYPICAL PHYSICAL DEMANDS:
- Occasional standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, and pulling.
- Occasional lifting and/or moving up to twenty-five pounds.

- Manual dexterity.
